Bug description: Most pool-type abilities apply permanent debuffs or incredibly long debuffs that cannot be removed, causing unecessary damage.

That's all I have screenshots of. However from my experience some other NPC's have those annoying debuffs:
Legion Hound
Guardian Construct
Seacursed Slaver
The Grimewalker

Also Habaron's shadow pools in maw of souls are spawned at maxed growth, even though the visual effect starts small and grows over time, the actual damaging radius is maxed from the moment they spawn and you can easily die in mythic 10+ like this. You basically have no time to get out of it without taking damage which is absurd. This might happen with other pool type abilities but im not sure.
